Fill out the Child Care Tax Credit Contribution Application

After completing the application, contribute to "St. Paul Child Care Center" & get the receipt from the church or school office.

Upload the CCTC-A form (application), and the CCTC-R form (receipt) given to you from the church office.

Save the CCTC-R as a receipt for your taxes.
